Medical & RX
Surest is a co-pay only plan with no deductible, featuring just an out-of-pocket maximum. It is not eligible for a Health Savings Account (HSA) as it does not qualify as a high deductible health plan. For more details, visit the Surest website (use code schreiber2025).
UMR offers two high deductible health plan (HDHP) options, both of which qualify for a Health Savings Account (HSA).
Regardless of the plan you select, each option:
- Includes prescription drug coverage through SmithRX
- Utilizes the UHC Choice Plus network
- Provides preventive care covered at 100%
- Protects you with an annual out-of-pocket maximum
- Offers enhanced care programs and resources to support your overall wellbeing

Prescription Drug Coverage
- Access Program: This program leverages additional savings on traditional branded medications, allowing members to have a low or $0 copay on prescriptions.
- Assist Program: No more pre-shopping for the best price or printing coupons! Your member ID has all the information that your pharmacy needs to find the best deals for you at the point of sale. When the pharmacy submits your claim, the Assist decision engine will find the lowest cost option for you, and that is what you pay at the counter.
- Access Plus Program: Many high-cost specialty medications can be accessed through other programs when a medication is not covered under the pharmacy benefit. SmithRx assists members in navigating and applying to these different programs.
