Insurance Options

Company Paid Insurance

Benefit-eligible partners are provided a company-paid life insurance benefit equal to two times their annual base earnings upon hire.

Supplemental Life Insurance

Partners have the option to purchase extra life insurance on themselves and dependent life insurance for their spouse and/or eligible children. Coverage levels and premium costs will be visible during enrollment.

  • When you enroll during a New Hire event, you won’t need to provide Evidence of Insurability for any level of supplemental coverage. You will be able to elect up to $50k in supplemental spouse life without evidence of insurability being needed. However, after your initial eligibility period, if you want to increase your coverage by more than one level or raise your spouse’s life insurance above $50,000, you will need to provide proof of good health through Evidence of Insurability (EOI). The required paperwork must be submitted and approved by Lincoln before the life insurance becomes effective and contributions are deducted from your payslip.
  • You can enroll in coverage for your covered dependent children, up to $20,000. Evidence of insurability is not needed for dependent children coverage.
  • If both you and your spouse work at Schreiber, you cannot buy spouse life insurance for each other. Only one of you can purchase life insurance on a shared dependent.

Accidental Death & Dismemberment (AD&D)

Accidental Death and Dismemberment (AD&D) insurance is a type of insurance that provides financial benefits if you or a covered dependent suffer a loss solely as a result of accidental injury that occurs while covered. For example, if you lose a limb or die because of an accident, AD&D insurance will pay out money to help you or your family. It doesn’t cover natural causes or illnesses, only accidents. This insurance is often chosen along with other life insurance benefits to give you extra protection. This benefit can be elected as single or family coverage.

Partners who elect life insurance for themselves and/or dependents may be subject to evidence of insurability (EOI), or proof of good health. First time enrollees may elect up to the maximum amount of supplemental life insurance coverage, the guarantee issue amount of spouse life ($50,000), and/or the maximum amount of dependent child life insurance without any EOI. During the annual enrollment period, partners can “bump up” one level of supplemental life insurance without EOI. Future elections for supplemental life insurance above the one level amount or spouse life above the guarantee issue amount ($50,000) are subject to EOI per plan guidelines. Coverage subject to EOI will not go into effect and payroll deductions will not be taken until the coverage has been approved.

Instructions to Submit Evidence of Insurability (EOI)

Step 1: Log in to MyLincolnPortal.com. If you are a first time user, register using company code: SCHREIBER

Step 2: Click on Complete Evidence of Insurability.

Step 3: Answer the required questions. You’ll be asked:

  • General applicant information, such as date of birth, height, and weight
  • Qualifying questions, including if you or other applicants have been diagnosed with a disease or are prescribed medications for a condition
  • Medical questions—if you or other applicants have a condition, Lincoln may need to know a little more about it, such as the name, diagnosis date, and treatments

Step 4: Electronically sign and submit the form.

Step 5: Save your confirmation report.

For questions regarding your EOI application, you may call (888) 287-8494, option 2.

Once you’ve submitted your application, Lincoln will process it as quickly as possible. In some cases, you may be auto-approved for coverage. If not, Lincoln will review your application and contact you if more information is required. In all cases, they will notify you of your application outcome. If additional medical information is needed, you will receive a letter by mail. You can check the status of your application online within 24 hours. Schreiber will also be informed of the final decision regarding your coverage.
