Dependent verification is a process to verify family members you enroll in benefits are eligible.

Dependent Verification

Did you Add Your Spouse or Your Child(ren) to Any Benefit Plan During Your Enrollment?

If yes, your enrollment is pending awaiting proof the dependents are eligible for coverage under the plan.

See below for the most common verification options. If you need assistance with your verification upload into Workday, contact your local HR.

Deadline for Spouse/Child(ren) Verification

New Hire/Life Event: Verify your spouse/child(ren) within 30 days, with your event date/date of hire being day 1 of the 30.

Open Enrollment: Check the Open Enrollment page or inquire with local HR for the spouse/child(ren) verification deadline.

Dependent Verification Options

Spouse

Married over one year
You can provide a copy of the first page of the most current tax year filed 1040 tax return (federal or state). The tax return should show ‘Married filing jointly’ or ‘Married filing separately’ and include both the partner and spouse’s names. You may black out SSN and financial data for privacy.

Married less than one year
You can provide a marriage certificate or a military ID for the spouse of the armed services member. The ID must be active and show both spouse and partner names and SSN (last 4).

Child

Natural or Stepchild
You can provide a birth certificate listing the partner and/or verified spouse as the birth parent.

Adopted Child
You can provide court paperwork such as an adoption placement agreement, petition for adoption, or adoption papers showing approval by the court with a visible court signature and/or court stamp. The paperwork should list the partner and/or verified spouse as the adoptive parent and include the name of the dependent/adopted child.

Legal Guardianship or Legal Custody
You can provide court-awarded legal guardianship/custody paperwork showing guardianship/custody granted under state law. The paperwork should name the partner and/or verified spouse as the child’s legal guardian/custodial parent, include the name of the dependent covered by the agreement, and show approval by the court with a visible court signature and/or court stamp.

  1. Log in to Workday and click on the Menu in the upper left corner.
  2. Select Personal Information, then click Worker Documents (in the View column on the right).
  3. Click Add, then Select files and choose the documentation you want to upload, then click Open.
  4. Choose Benefits – Dependent Verification when verifying dependents and Benefits – Life Event when verifying a life event.
  5. Add a description in the comment box to keep track of your uploads. Example: Ron Birth Certificate
  6. To load another document immediately, click upload. If you are done uploading documents click OK, and then click Done.
  7. If you need to add more documents, begin again from step number 3.

Child eligibility for benefits ends at age 26. No enrollment changes are necessary by the partner. Workday will automatically remove overaged children from coverage the last day of the month they turn 26. A COBRA continuation packet will be mailed to the address listed in Workday.
