Supplemental Plan Options
Hospital Indemnity (HI)
Hospital indemnity insurance provides extra financial support in addition to your regular health insurance benefits. This coverage is especially useful if you or a covered dependent has an accident that leads to hospitalization. You can find coverage amounts and bi-weekly costs in Workday during enrollment. Critical Illness (CI)
Schreiber offers critical illness insurance that pays a financial benefit if you receive a serious medical diagnosis covered by the plan. The benefits are paid directly to you and can be used to help cover your annual deductible, coinsurance expenses, or any other costs. The payout is not dependent on other health care benefits. Covered diagnoses include heart attack, stroke, cancer, major organ failure, MS, and more. You can purchase coverage for yourself or your spouse, and if you elect coverage, your children will automatically be enrolled at 50% of your coverage. Voluntary Accident (VA)
Voluntary accident insurance is a benefit that provides extra financial support on top of your existing health insurance benefits if you or a covered dependent is involved in a qualifying accident. Earn Rewards with UNUM’s Be Well Benefit
The Be Well Benefit offers a $100 health incentive for covered individuals who complete a routine health screening each year. Each benefit on this page qualifies for the Be Well Benefit, meaning you could earn up to $300 if you enroll in Critical Illness, Voluntary Accident, and Hospital Indemnity plans and complete a routine health screening. Dependents covered under these plans are also eligible for the $100 Be Well Benefit. Additional benefit information can be found here.
Eligible Screenings to Earn the Be Well Benefit
- Annual exams by a physician including sports physicals and well-child visits, dental and vision exams
- Cancer screenings including pap smear, colonoscopy
- Cardiovascular function screenings
- Cholesterol and diabetes screenings
- Imaging studies, including chest X-ray, mammography
- Immunizations including HPV, MMR, tetanus, influenza
Be Well Benefit Submission Instructions
You Can Receive the $100 Benefit for Tests That are Performed After Your Initial Coverage Date
Step 1: Complete a qualified health screening test.
- Annual exams by a physician including sports physicals and well-child visits, dental and vision exams
- Cancer screenings including pap smear, colonoscopy
- Cardiovascular function screenings
- Cholesterol and diabetes screenings
- Imaging studies, including chest X-ray, mammography
- Immunizations including HPV, MMR, tetanus, influenza
Step 2: File your claim online at login.unum.com, MyUnum for Members app, or over the phone at 1-800-635-5597.
- If filing for the first time, click Create an account. We recommend using a personal email address that is easily accessible when away from work.
- You will be asked to enter your birthdate and SSN
- A one-time security code will be sent to the email you provide
- If you already have an account, enter your email.
Step 3: Once you’re logged in, begin with Start a Claim or Leave then Continue.
Step 4: From the drop down, choose who the claim is for (Yourself, Spouse, or a dependent), then choose Wellness/Health Screening and follow the prompts.
Important: Have the following information ready to submit the claim:
- First and last names of the partner and claimant (the partner might not be the claimant)
- Partner’s Social Security number or policy number
- Name and date of the test
- Name of physician and the facility where the test was performed
HI, CI, VA Claim Instructions
- If filing for the first time, click Create an account. We recommend using a personal email address that is easily accessible when away from work.
- You will be asked to enter your birthdate and SSN
- A one-time security code will be sent to the email you provide
- If you already have an account, enter your email and login
- Is sick, critically ill, needs surgery or in hospital would be a Critical Illness or Hospital Indemnity claim.
- Had an accident or injury would be a Voluntary accident claim.
- Your employment information
- Medical care resulting from the event, like surgery
- Medical providers visited—physicians, hospitals, other medical professionals, etc.
- Confirm responses
- Provide medical authorization
- Review fraud statement
- Select Accept and Submit
- View confirmation screen, see any next tasks and track progress
