Carrum Health

Carrum Health is a Centers of Excellence network that negotiates directly with top-quality healthcare providers to provide medical plan participants with nationwide access to the very best specialty care. Whether you need cancer care, behavioral health treatment, or a range of surgical procedures, Carrum Health works with the top cancer specialists and surgeons in the country to facilitate better outcomes and superior bedside manner. Plus, to make the journey less stressful, a Carrum Care Specialist will manage the entire process, so you don’t have to. For additional benefit details, view the Summary Plan Description here.

Carrum Health offers a range of treatments through its Centers of Excellence (COE), which include surgeries, substance use disorder treatments, and cancer care. Treatments provided by Carrum Health.

Surgery:

  • Total, partial, and revision major joint replacement surgery (e.g., hip, knee, shoulder, ankle)
  • Spinal (back and neck) surgery (e.g., spinal fusion, decompression, disc replacement)
  • Bariatric (weight loss) surgery and revision.
  • Minor orthopedic procedures (e.g., surgeries of the hand, wrist, elbow, shoulder, ankle, foot) other than major joint replacements*
  • Cardiac (heart) surgery (e.g., bypass, valve repair and replacement)
  • Gynecologic surgery (e.g., hysterectomy)

* Only available for participants who reside within 150 miles of the nearest COE

Substance Use Disorder treatment:

  • Detox
  • Residential treatment
  • Partial hospitalization (PHP)
  • Intensive outpatient (IOP)
  • Outpatient including virtual

Cancer care:

  • Advisory Program (virtual second opinions / expert reviews and support)
  • Treatment (e.g., excision surgery, chemotherapy, radiation treatment, supportive care such as psychosocial, nutrition, and family support)

Carrum Health is available to partners and dependents (18+ years of age) who are enrolled in a Schreiber medical plan.

High Deductible Health Plans (HDHP) :
For partners enrolled in a High Deductible Health Plan (HDHP), surgery costs are fully covered once you meet the IRS minimum deductible. This means that after spending $1,650 towards qualified medical or prescription expenses for employee-only coverage, or $3,300 for all other tiers of coverage, your entire procedure will be covered at 100%. Carrum coordinates with your health plan to verify your remaining balance, when applicable, prior to surgery.

Surest Copay Plan:
Surest members will have a $0 copay when utilizing Carrum for services.

Travel:
Expenses incurred for transportation, lodging, meals, and incidentals are covered for the participant and one adult companion as long as travel arrangements are scheduled and reserved through Carrum Health. A daily allowance will be paid to the participant prior to travel to the Carrum provider, to be used at the discretion of the participant and companion

Plan participants can contact Carrum Health at 1-888-855-7806, Monday-Friday 9am-8pm EST, online at carrum.me/schreiberfoods, or by downloading the Carrum Health app on iPhone and Android devices to search for and compare participating Centers of Excellence.

After contacting Carrum Health, a participant is assigned a Care Specialist. The Care Specialist services can include assistance with selection of a Center of Excellence, medical records collection, appointment scheduling, and travel reservations and logistics management. The Care Specialist can also assist the participant with registration for the Carrum Health Benefit through the Carrum Health app and completion of required forms. Participants are required to agree to Carrum Health’s Terms of Service and Member Registration Agreement and must also agree to provide their medical records and any other relevant information to their selected Center of Excellence as needed to schedule a consultative evaluation. Medical records and images may be collected on behalf of participants by their assigned Care Specialists. During the consultation, the Center of Excellence will determine if the participant is an appropriate candidate for the requested procedure or treatment. Receiving this consultation does not commit a participant to proceed with the procedure or treatment, or to use the Carrum Health Benefit.

Eligibility

Partners and their dependents enrolled in a Schreiber medical plan.

How to Get Started

  • Step 1: Activate your account by visiting https://www.2nd.md/schreiber, download the 2nd.MD app, or call 1.866.269.35341.
  • Step 2: Get connected with an experienced 2nd MD nurse. They’ll ask you to explain your medical concerns, and will handle the rest, including collecting medical records and connecting you with a leading specialist who is an expert in your condition.
  • Step 3: Connect with a board-certified, elite specialist for a virtual expert medical consultation via phone or video from the comfort of your home.
  • Step 4: After your consultation, you’ll receive a written summary of your consultation so you’re prepared for a conversation with your treating doctor or we can refer you to another in-network doctor in your area.
